PrEP 101 โ PrEP vs PEP
The difference, in plain EnglishPrEP vs PEP: which one do you need?
They sound alike and both prevent HIV, but they're used at opposite moments. PrEP is the seatbelt you wear before the journey; PEP is the airbag after a crash. Here's the difference in one table โ and how to know which you need right now.
PrEP vs PEP at a glance
| PrEP (pre-exposure) | PEP (post-exposure) | |
|---|---|---|
| When you take it | Before & during ongoing risk | After a possible exposure |
| Deadline to start | Any time โ planned | Within 72 hours (sooner is better) |
| How long | Ongoing (pill or injection) | A 28-day course |
| Best for | Repeated or ongoing risk | A one-off / emergency |
| Effectiveness | ~99% (sex), as prescribed | High if started fast & completed |
| Prescription & testing | Yes โ routine follow-up | Yes โ urgent, plus follow-up tests |
Decide in 10 seconds
- You might have been exposed in the last 72 h and you're not protected โ you need PEP, urgently. Go to an ER or sexual-health clinic.
- No specific exposure, but you have ongoing risk (new/multiple partners, a partner with HIV who isn't undetectable, inconsistent condoms, injection drug use) โ PrEP is your long-term protection.
- It's been more than 72 h since a worrying exposure โ PEP won't work now, but see a provider promptly to test and start PrEP going forward.
- Just finished PEP and still at risk? โ switch to PrEP with no gap (below).

From PEP to PrEP โ no gap
If you needed PEP, you clearly have some risk โ so many people move straight from a 28-day PEP course onto ongoing PrEP. Your provider can bridge them seamlessly ("PEP-to-PrEP") so you're never unprotected. It's a smart moment to switch from reacting to exposures to preventing them.
PrEP vs PEP โ FAQ
Can I take PEP instead of PrEP regularly?
No โ PEP is a one-off 28-day emergency course, not for repeated use. If you keep needing it, PrEP is the right tool.
I'm on PrEP โ do I ever need PEP?
If you take PrEP correctly, you're already protected and don't need PEP. PEP is for people not on PrEP (or with major missed doses).
Is it too late for PEP?
PEP must start within 72 hours. After that it doesn't work โ but see a provider to test and start PrEP.
Are the medicines the same?
They overlap (both use tenofovir/emtricitabine), but PEP adds a third drug and is a fixed 28-day course, while PrEP is ongoing and can be a pill or injection.
