Descovy for PrEP: side effects, weight & cost
Descovy is the newer, “gentler” daily PrEP pill. Its smarter form of tenofovir means less kidney and bone impact and less early nausea than Truvada — with two trade-offs to know about: modest weight and lipid changes, and that it is not approved for vaginal sex.

When does Descovy start working?
- Anal sex: protective after about 7 days of daily dosing.
- Vaginal / front-hole sex: not established — Descovy is not approved for this exposure. Use Truvada, Apretude or lenacapavir instead.

Why would I choose Descovy over Truvada?
Descovy is gentler on the kidneys and bones and causes less early nausea. It’s a strong choice if you have kidney concerns, low bone density, are over 50, or couldn’t tolerate Truvada’s stomach effects. The trade-offs are cost, some weight/lipid change, and no vaginal-sex approval.
Is the weight gain a big deal?
It’s usually modest (about 1–3 kg on average) and varies a lot between people. It matters most if you already have weight, diabetes or heart-disease risk. Track your weight and lipids; GLP-1 medicines and lifestyle changes can offset it if needed.
Why isn’t Descovy approved for vaginal sex?
The approval trial (DISCOVER) only studied anal-sex exposure, so there isn’t evidence for receptive vaginal sex. Rather than assume, regulators limited the approval. If you have vaginal exposure, Truvada, Apretude or lenacapavir are the approved options.
Does Descovy interact with other medicines?
TAF has fewer kidney-related interactions than TDF (for example, NSAIDs are less of an issue). But strong enzyme inducers (rifampin, some seizure drugs, St. John’s Wort) can still lower it. Check yours in the Drug Interaction Checker →
