"PrEP makes you gain weight" is one of the most common worries — and it's mostly a myth for Truvada, with a grain of truth for Descovy. Here's what the evidence actually shows across every PrEP option, and what you can do about it.

The headline: for the daily pills, the difference between Truvada and Descovy is real but small — a few kilograms on average, not a dramatic change.
Why the "PrEP = weight gain" story spread
Much of the concern comes from treatment studies (people living with HIV) where switching from older drugs to TAF-based regimens with integrase inhibitors showed weight gain — a different situation from PrEP. For prevention, the nuance is simpler:
- TDF slightly suppresses weight. So when people switch from Truvada to Descovy, losing that suppression can look like the new drug "causing" gain.
- Appetite returns. Feeling healthier and less anxious on PrEP can naturally increase appetite — a good thing that's easy to misattribute.
- Life happens. Weight drifts up with age and lifestyle regardless of PrEP; correlation isn't causation.
What to do if you're worried about weight
- Weigh the trade-off with your provider. If weight/cholesterol matters to you, Truvada may be preferred over Descovy — as long as your kidneys and bones are healthy.
- Check your baseline. Ask for weight and a lipid panel when you start, so any change is real data, not a guess.
- Don't stop PrEP to "test" it. Switching keeps you protected; stopping doesn't.
- Usual basics still work — movement, sleep, and diet influence weight far more than PrEP does.
- On a GLP-1 (Ozempic/Wegovy) or other meds? No PrEP interaction — confirm anything in the drug checker.

Does PrEP cause weight loss?
Some people on Truvada (TDF) notice slight weight loss or appetite change, because TDF has a mild weight-suppressing effect — the opposite of the modest gain sometimes linked to Descovy (TAF). Large weight changes usually have another cause; mention them to your provider.
How much weight might Descovy add?
On average roughly 1–3 kg, with a small rise in cholesterol. Individual results vary and many people notice nothing.
Is Truvada better for staying lean?
It's weight-neutral to mildly suppressing, so it's often chosen when weight or lipids are a concern — provided kidneys and bones are fine.
Do the injections cause weight gain?
Cabotegravir and lenacapavir look largely weight-neutral in PrEP use, without a strong weight signal.
Will switching PrEP reverse the weight?
It may modestly, but weight has many drivers. Discuss realistic expectations with your provider before switching.
