U=U: Undetectable = Untransmittable

When the partner living with HIV takes treatment and keeps an undetectable viral load, they cannot pass HIV on sexually — full stop. This isn't "low risk," it's no risk for sexual transmission, confirmed by large studies (PARTNER/PARTNER2) that saw zero transmissions from an undetectable partner.

What "durably undetectable" means: an undetectable viral load on treatment, confirmed and maintained for at least 6 months, with good adherence and regular monitoring.

So — do you (the negative partner) need PrEP?

If your partner is durably undetectable, U=U already protects you. PrEP is worth adding in these situations:

Is U=U really 100% for sex?

Yes, for sexual transmission when the partner is durably undetectable. It does not prevent other STIs, and it isn't about needle-sharing — but for sex between partners, undetectable means untransmittable.

My partner just started treatment — should I be on PrEP?

It's a sensible bridge until "undetectable" is confirmed (usually within ~6 months). Talk to a provider about starting PrEP now and reassessing later.

Can we conceive without PrEP?

Yes, when the partner with HIV is durably undetectable, timed condomless sex is effectively zero-risk. PrEP simply adds reassurance and covers adherence gaps.

What if we slip up and I'm worried about an exposure?

If the partner is undetectable, there's no transmission risk. If there's genuine uncertainty and it was under 72 hours ago, see PEP.
