How does on-demand 2-1-1 PrEP work?

2-1-1 (event-driven) dosing is for anal sex in men who have sex with men: take 2 pills 2–24 hours before sex, then 1 pill 24 hours after the first dose, and 1 pill 48 hours after. It uses Truvada (TDF/FTC) only. It is not FDA-approved in the US (standard in Europe, Canada and Australia) and is not for vaginal sex.

How often should men on PrEP test for STIs?

PrEP doesn't prevent other STIs, so testing every 3 months is recommended — ideally at all exposed sites (throat, rectum and urethra), since many throat and rectal infections have no symptoms.

Can I use poppers and Viagra on PrEP?

Both are fine with PrEP itself, but NEVER combine poppers (nitrites) with Viagra/Cialis (PDE5 inhibitors) — together they can cause a fatal blood-pressure drop. Check any medicine in our drug-interaction checker.

PrEP doesn't stop STIs — here's the routine that does the rest

Test every 3 months — at the throat, rectum and urethra, not just urine (most throat/rectal infections have no symptoms).

The drugs actually worth watching

Poppers + Viagra/Cialis — fine with PrEP separately, but never together (dangerous blood-pressure drop). This risk is independent of PrEP.

Is on-demand 2-1-1 as good as daily PrEP?

For anal sex in men, event-driven dosing was highly effective in the IPERGAY trial. The catch is timing — the "2" pre-dose must be 2–24 hours ahead. It's ideal for plannable sex; daily or an injection is simpler if sex is frequent or spontaneous.

How often do I really need to test?

Every 3 months for HIV and STIs (3-site), plus kidney checks on tenofovir. It's quick and part of every PrEP visit.

Can I use PrEP if I party / chemsex?

Yes — and an injectable can help if drug use makes daily pills unreliable. Be honest with your provider; it won't affect your access, and hydration + adherence matter more when using stimulants.

Poppers and PrEP?

Safe with PrEP. The only hard rule: never mix poppers with Viagra/Cialis or nitrate heart meds.
