HIV doesn't retire — new diagnoses in people over 50 are rising, partly because PrEP is offered to older adults far less often. The good news: PrEP works at any age. The only real nuance is picking an option that's kind to aging kidneys and bones, and fitting it around the medicines you already take.
Why the choice matters more after 50
Two things change gradually with age: kidney function declines, and bone density drops. One PrEP ingredient — tenofovir disoproxil (TDF), in Truvada — has a small effect on both. The newer forms don't. So the choice is usually about matching PrEP to your kidney and bone health, not about whether PrEP is "for you."

PrEP and the pills you already take
- No interaction with common blood-pressure meds (ACE inhibitors, ARBs, amlodipine), statins, or most heart drugs — several BP meds are even kidney-protective.
- On Truvada, the one to watch is regular NSAID painkillers (ibuprofen, naproxen) — they add kidney strain, so prefer acetaminophen or switch PrEP.
- Diabetes, thyroid, reflux and prostate medicines are generally fine — check every one in our 112-medicine checker.
- Keep your kidney check (and a bone scan if on TDF with risk factors) on schedule.
PrEP for older adults — FAQ
Am I too old for PrEP?
No — there's no upper age limit, and HIV risk doesn't stop with age. If you're sexually active and could be exposed, PrEP is appropriate.
My kidneys aren't perfect — can I still take PrEP?
Often yes, by choosing Descovy or an injection, which barely touch the kidneys. Your provider will check eGFR to guide the choice.
I take a lot of medications — is that a problem?
Rarely. PrEP has few interactions. The main one on Truvada is regular NSAIDs. Run your list through the checker.
Will PrEP weaken my bones?
Only Truvada (TDF) causes a small, usually reversible dip. TAF and the injectables are bone-neutral — a good pick if you have osteoporosis.
